Генетические модификаторы фенотипа спинальной мышечной атрофии

Проксимальная спинальная мышечная атрофия (СМА) – аутосомно-рецессивное нейродегенеративное заболевание, обусловленное гомозиготной делецией 7-го экзона гена SMN1. Цель работы: анализ ассоциации аллельного полиморфизма теломерных генов SMN1, NAIP и центромерного гена SMN2 области 5q13 с клиническим фенотипом СМА. Показано, что гомозиготный генотип с теломерной делецией, которая охватывает не только SMN1, но еще и ген NAIP, достоверно чаще наблюдается у пациентов с самым тяжелым типом СМА. Три и более копии SMN2 ассоциировано с более легким фенотипом, количество копий SMN2 намного сильнее влияет на фенотип СМА, чем протяженность теломерной делеции. Установлено, что у СМА-пациентов с гомозиготной делецией SMN1 и NAIP достоверно чаще выявляют 1 копию SMN2. Это может свидетельствовать о наличии большой делеции, охватывающей все три исследуемых гена и ассоциированной с тяжелым типом СМА. Отмечено, что врожденная СМА (0 тип) достоверно реже встречается у девочек, что может указывать на наличие генов модификаторов СМА на Х-хромосоме.

РЕЗЮМЕ. Проксимальная спінальна м’язова атрофія (СМА) – аутосомно-рецесивне нейродегенеративне захворювання, обумовлене гомозиготною делецією 7-го екзона гена SMN1. Мета роботи: аналіз асоціації алельного поліморфізму теломерних генів SMN1, NAIP і центромерного гена SMN2 ділянки 5q13 з клінічним фенотипом СМА. Показано, що гомозиготний генотип з теломерною делецією, в якій залучено не тільки ген SMN1, але й ген NAIP, достовірно частіше спостерігається у пацієнтів з найважчим типом СМА. Три і більше копії SMN2 асоційовані з легшим фенотипом, кількість копій SMN2 набагато сильніше впливає на фенотип СМА, ніж протяжність теломерної делеції. Встановлено, що у СМА-пацієнтів з гомозиготною делецією SMN1 і NAIP достовірно частіше виявляють 1 копію SMN2. Це може свідчити про наявність великої делеції, яка охоплює всі три досліджуваних гена і асоційована з важким типом СМА. Відзначено, що вроджена СМА (0 тип) достовірно рідше зустрічається у дівчат, що може вказувати на наявність генів модифікаторів СМА на Х-хромосомі.
